Be sure to fill out all necessary forms before your visit. We have listed them for you: Patient Forms to Fill Out.
Additionally, bring all radiology films (x-rays, MRI, scans, etc) and your medical records to your first visit. Do not rely on the doctor's office to deliver the films or records. We suggest you pick them up from your physician and hand carry them to your appointment. If you are unable to bring your films, we will have to reschedule the appointment.
